日気食会報,73(3),2022Two Cases of Basaloid Squamous Cell Carcinoma of the Hypopharynx Treated Using Endoscopic Laryngopharyngeal SurgeryJ. Jpn. Bronchoesophagol. Soc., 73 : 222─230, 2022Basaloid squamous cell carcinoma (BSCC) is a rare variant of squamous cell carcinoma (SCC) with a basaloid component and squamous cell differentiation that is located submucosally. Diagno-sis prior to surgery is difficult because of its location and mixed histological features. Here, we re-port two cases of BSCCs resected using endoscopic laryngopharyngeal surgery (ELPS), which is a minimally invasive endoscopy-assisted surgery for laryngopharyngeal tumors that uses a curved laryngoscope and curved instruments. The first case was a 64-year-old man who had undergone brachytherapy and neck dissection for treatment of an oropharyngeal SCC five years earlier and transoral resection for treatment of another oropharyngeal SCC one year earlier;the patient was diagnosed as having hypopharyngeal SCC during a regular checkup. The tumor was resected us-ing ELPS, and pathological examination revealed a BSCC. This patient dropped out of follow-up care, but no local recurrence was observed prior to dropout. The second case was a 67-year-old man who had undergone endoscopic submucosal resection for treatment of a gastric cancer;the patient was diagnosed as having pharyngeal cancer during a follow-up endoscopy. The tumor, which was initially diagnosed as SCC based on a biopsy, was resected using ELPS, and the final pathological examination revealed BSCC with a positive margin. Adjuvant radiotherapy was per-formed, and no local recurrence has been detected to date. Recently, more small tumors are being detected thanks to improved endoscopic technology. BSCC is an important differential diagnosis even though it can be difficult to diagnose accurately prior to surgery.
