J. Jpn. Bronchoesophagol. Soc., 73 : 203─209, 2022Unresectable esophageal cancer is often associated with stenosis and fistula formation, leading to quality of life deterioration. Recently, minimally invasive stent placement is often selected for such cases, and opportunities to perform bypass surgery have been decreasing. Among 20 pa-tients who underwent bypass surgery in our institute, the median length of hospital stay was 19 days (12─62), and no hospital death was observed. Postoperative pneumonia was associated with preoperative fistula formation and a prognostic nutritional index (PNI) below 40 (p=0.07). Two of the three cases with anastomotic insufficiency had a low modified Glasgow prognostic score(mGPS) of 2 (p=0.06). Careful perioperative management is required in cases with preoperative fistula or cases with a PNI value under 40.2014.17) 舟木 洋,藤井 頼,藤田 純・他:切除不能進行食道癌に対する栄養評価からみた食道バイパス術の検討.癌と化学療法42:1543─1546, 2015.18) Kimura M:Bypass operation for unresectable esophageal cancer:postoperative complications Short-term Outcomes of Esophageal Bypass Surgery for Patients Key words : unresectable esophageal cancer, bypass surgery, postoperative complicationafter thoracotomy versus no thoracotomy.
